You will be contacted to confirm your appointment prior to the scheduled date. Please respond to one of the confirmation contacts.
Bring your completed New Patient paperwork. Also, bring picture ID and your insurance card(s) to your appointment.
Be sure to plan to arrive about 15 minutes before your scheduled appointment time for registration.
Please allow additional time for parking and the construction on Central Ave. Valet Parking is available at the Presbyterian Hospital entrance, for your convenience.
If you need to reschedule or cancel your appointment, please provide as much notice as possible. Call at least 1 day prior to the scheduled date, to avoid cancellation fees.
